Services for people with severe mental illness

Mental Health Hubs

Mental Health Hubs make it easier for people with severe mental illness to access integrated clinical and non-clinical care across the North Brisbane and Moreton Bay  region.

Each hub is operated by a different provider with extensive experience in providing services to people with severe and complex mental illness.

Mental health hubs offer a range of services:

  • assessment, triage and intake
  • navigating services
  • coordinating care
  • psychological therapy groups
  • physical health care
  • psychosocial support
  • accessing the NDIS
  • partnering with other service providers (e.g. alcohol and other drug treatment, employment, housing, finance)
  • outreach services.

Eligibility criteria

  • 18 years and over
  • have a diagnosable or severe and complex mental illness (episodic or persistent)
  • experiencing significant disruption to daily life, wellbeing and functioning
  • unable or unwilling to access NDIS
  • not accessing services through public hospitals
  • in need of clinical support that is not being provided by NDIS or hospital services.

Please note that people who are experiencing acute mental illness should continue to be referred to Metro North Mental Health Services via 1300 MH CALL.

If you're a loved one of someone who needs support, you can refer them to the mental health hubs as long as you have their consent.

Referrals can be made directly with the provider or by contacting the My Mental Health Service Navigation Team on 1800 752 235.

GPs can refer by completing a referral via the GP RediCASE Integrator or via HealthPathways.
